IRB Submission Quick Steps
*To ensure the accuracy of each IRB submission, the person(s) submitting for approval must check each of the appropriate boxes. A submission will not be reviewed for implementation until the checklist is complete. Please include the completed checklist with each submission as a table of contents. Check List.

Step 1 – Complete CITI Training and Submit Certificate

Step 2 – Submit Research Application and Protocol Form
If you are a UNA Student or Faculty Member, you must use your UNA portal email for all communications and use on all applicable forms (application, proposal, consent/assent forms, etc.). -
Step 3—Complete and submit any other required documentation, such as the Assent and Consent Forms (below), copies of surveys, etc.

Step 4—Submit all forms electronically by self-enrolling on the . Once enrolled, locate the module entitled "Submission Portal Module," click on the assignment labeled "IRB proposal submission," and upload all of the proposal documents into Canvas.
